Lich's Eye Bullets is a passive item. The Gunslinger starts with this item.

Notes
- Certain synergies will not be simulated with Lich's Eye Bullets, or have additional requirements for being simulated.
- Some two-component synergies that require both guns/items to function, as opposed to one or the other simply being in the player's inventory, will not be simulated. Examples and exceptions include:
- Dual-wielding synergies such as
The Submachinist will not be simulated. However, if a dual-wielding synergy also gives additional benefits, such as with
Trigger Twins, the bonuses will still be granted.
Rubenstein's Monster will not be simulated.
- The 38 Special's
Detective Mode synergy would still need the player to acquire the Badge for the policeman to show the contents of chests.
- If the player has Jolter,
There Is Only War will be simulated and the player will have a floating Demon Head while holding Jolter even if the player does not also have the Demon Head.
- Dual-wielding synergies such as
- Synergies that require 3 or more guns/items will only count Lich's Eye Bullets as one of the synergy's components, and more will need to be collected for the synergy.
Resourceful Indeed will only be simulated if Elimentaler and two of the synergy's other items are acquired.
- Synergies that cause guns to transform may override the effects of other synergies. Examples include:
- With Big Iron,
Cast Iron is overridden by
Iron Slug.
- With Lower Case r,
Just In Case overrides all of the gun's other synergies.
- With Big Iron,
- Some two-component synergies that require both guns/items to function, as opposed to one or the other simply being in the player's inventory, will not be simulated. Examples and exceptions include:
Trivia
- Possible reference to Clint Eastwood's "Deadeye" as the Lich is the lord over the dead.
- Nod to the fact that The Gunslinger & Lich both have bullets as prosthetic eyes.
- Due to this, the Gunslinger is unable to drop Lich's Eye Bullets.
- Possible reference to Urza's Eyes from Magic: The Gathering; two powerstones acting as the source of his near omnipotence.
- This item appears on the win screen after defeating the Lich, in a pile of ashes.
- Possibly a reference to Acererak the Demilich from Dungeons & Dragons, who has magic soul gems instead of eyes.
